We are pleased to share Bridging Gaps in Care: A Population Health Approach for Primary Care Residents. This free course is designed to provide residents with resources for their QI Project, including 8 modules and an embedded QI Toolkit. Designed in partnership with UNM and NMSU, family medicine residents will be equipped with the skills needed to recognize and respond to the social and structural factors that shape the health of their patients and communities.
As an incentive for completing the modules and providing feedback after each section, family medicine residents in New Mexico and Colorado will be eligible to receive a $200 stipend.
Course Overview:
Module 1: Foundations of Population Health
Module 2: Understanding patient health-related social needs
Module 3: Collaborating to address patient health-related social needs
Modules 4: Care of populations that have been economically and social marginalized
Module 5: Addressing community social and structural needs
Module 6: Community assessment and relationship building
Module 7: Policy and advocacy (optional)
Module 8: Final Assessment
Modules include lessons, activities, case studies, clinical pearls, reflection questions, knowledge checks, additional resources or readings, and Learning Experience Evaluations. The embedded toolkit, located in module 3, is the QI Essentials Toolkit by the Institute for Health Care Improvement. Using the QI toolkit ensures eligibility for the $200 incentive.
